Congratulations!  Your company has decided to integrate Patient Advantage services into your wellness program.  You now have access to a host of unique programs designed to help you become the healthiest version of you!

 

If you haven’t received it already, you’ll soon be getting your login information to the Patient Portal from your HR department.  When you access the Patient Portal you can find wellness programs your company is currently conducting and enter your own wellness stats into your profile to help track your health and fitness goals.

New Resources Available to You

You may be asking, what can I expect from Patient Advantage?  Is this going to be like all the other healthcare services I’ve tried in the past? What makes Patient Advantage different?

We believe that healthcare is personal. This is one of the reasons we consider our team of highly trained Nurse Advocates as a front-line resource for you and your coworkers.

Nurse Advocates

Have you met your Nurse Advocate yet?  If not follow this link to find out a more about our team of Nurse Advocates.

Usually, our Nurse Advocates visit the work place twice a month to meet employees (like yourself) and address any health issues they may be facing.  Our Nurse Advocates also take blood pressure readings during their bi-monthly visits. 

No need to worry, this test is totally voluntary and will not be used with any identifying information (such as your name or employee number) when we report our findings to your employer.  We use these readings to show your employer health trends amongst groups of their employees’ over time.  Your data set will never be singled out and no one will ever know what categories you fall into.  All participant data is kept totally anonymous.

Here at Patient Advantage our Nurse Advocates have proven (through blood pressure readings and other health and wellness metrics) that having someone available to answer questions about health concerns, medication, and wellness goals leads to healthier lives with less catastrophic incidents.

What is a catastrophic incident? 

Your doctor prescribes a new blood pressure medication for you.  You believe you understand what you should be doing, how you should be taking the medication, but you’re not completely sure.  Turns out you’ve been taking the medication incorrectly and you wind up spending the day in the ER or an urgent care facility.  Not only does this end up costing you time and money but also causes you undue stress, both emotional and physical. 

How could this have been prevented? How can we keep this from happening again?

Here at Patient Advantage we believe that you should be spending your free time doing things you enjoy, with people you care about.  We’re here to help make that your reality.

If you ever have any questions about your medications, need help obtaining the right medications, have a health issue you’re struggling to deal with, or one of your family members has a health issue that you would like some guidance on – please reach out to your Nurse Advocate.  You will be able to find them in your workplace (usually a cafeteria or break room) or you can get in touch with them now by checking out our Nurse Advocate page.

Patient Portal

The Patient Advantage Patient Portal was designed to make it easy for you to navigate our website and to use the services we offer.  The Patient Portal is set up to act as your one stop shop for all the information you need. 

When you first log into the Patient Portal you’ll be asked to create a unique password.  No one will have access to your new password, and no one will be able to access your account.

Next, you’ll be prompted to set up your profile.  We ask for some sensitive information such as your age and weight.  You can enter as much or as little information as you feel comfortable with.  However, as you add more information about yourself, it becomes easier for us to provide you with any additional assistance you may need.  Also, if you do choose to enter data about yourself, you’ll be able to track your own progress over time.

You can reach your goals - we can help.

Patient Advantage is compliant with all HIPPAA regulations, which means all the data you give to us will be completely confidential. We promise not to share your data with anyone outside of our team unless you grant explicit permission.  This also means we will not be sharing any identifiable information with your employer.  We will use group data sets to show how our programs are working towards creating a healthier workforce, but there will be no way for your employer to gain access to your specific health information.

Once your profile is complete, you’ll find yourself on our navigation page.  From here you’ll be able to access the different health and wellness programs your company is running, enter updates on your datasets (ex. Updating your weight or blood pressure readings), and gain access to our video library.

When you’re on the Navigation page you can hover over one of our menu items for a short description of where the link will take you.  By clicking on the link you’ll be transported to the next page.  If you want to return to the Navigation page, click on the home button and you’ll find yourself back where you started.

We will compile a 50 to 100-page report for you that will break down your diagnosis into simpler, easier to manage chunks.  Then your Nurse Advocate will go through the report with you (at the work site or in your home) helping you to understand all the information your report contains.  If you have any additional questions your Nurse Advocate will be there to listen, answer and help as best they can.

Health Program Initiatives

Maybe you’re not having a dire medical emergency, but you’d like assistance in improving the quality of your healthcare and self-care.  Patient Advantage wellness programs can help you to meet your goals and become the healthiest version of you. 

Our Nurse Advocates will act as your Health Coach, guiding you past the hurtles that have kept you from changing your unhealthy habits and behaviors.

Our nurses are well versed in various nutrition and exercise regimes.  You can also get in on the Patient Advantage Challenge, a weight loss program you can participate in with your coworkers and loved ones.  Your Nurse Advocate will be there to guide you and you’ll have a built-in support system - since you’ll be completing the program together. One Patient Advantage member who completed the Patient Advantage Challenge lost over 100 pounds! The next big loser could be you.

We also offer a smoking cessation program, because no one should have to quit alone.  A recent study found that regular tobacco users try to quit an average of three times before they’re successful. However, each time you set your goal to quit smoking and strive towards becoming a quitter, the more likely you are to reach your goal.  What does this mean? If you’ve tried to quit in the past and failed, don’t be too hard on yourself.  The fact that you made the effort and will continue to do so means that you have a greater chance of success than when you started.  Congratulations on working towards becoming a quitter.  We hope our smoking cessation program will help you be the quitter we know you can be.
